Someone will contact you within 48 hours to schedule an appointment. Please complete the New Patient Questionnaire and download the appropriate paperwork for your appointment from below.

Appointments for Current Patients
Please contact us to schedule an appointment and complete the Follow-up Questionnaire before your visit.
- To schedule an appointment you may send us a secure message on our patient portal, or you may call 301-654-1575 and select option 6.
- The phones are answered between 9 AM – Noon and 2 – 4 PM. Scheduling is time consuming and the staff is on the phone constantly and can only speak with one patient at a time. If the staff member is free the phone will ring through and be answered. If they are on the phone then your call will roll over into our voice mail system. Please leave a detailed message including your name, contact phone numbers, referring doctor and the reason for your request. Your call will be returned within one working day.
- Before coming in for your appointment, please complete the online follow up questionnaire.
- Please allow additional time, about 20 minutes, if you plan to park in the Barlow Building parking garage. Construction has drastically slowed the parking process.
Referrals for New and Current Patients
There are many different insurance companies each with a multitude of different plan and benefit packages. It is your responsibility to understand your insurance policy. If your insurance plan requires that you have a referral from your primary care physician in order to see a specialist or to have a study then you must have the referral available at the time that you call to schedule and you must bring it with you to your visit. Acquisition of the referral is your responsibility. If you arrive for a visit without a proper referral you will not be seen and will be responsible for a late cancellation fee, unless you opt to be seen on a fee-for-service basis outside the auspices of your insurance company and pay for your visit. Please be responsible and obtain your referral on a timely basis and be sure to bring it with you.
