Consultation

 

Your doctor has referred you to The Center for Sleep & Wake Disorders for a clinical consultation. The purpose of the consultation is to understand your complaint about your sleep and/or wakefulness and develop a diagnostic and treatment plan. Prior to coming in for your appointment you please complete the Full Sleep Questionnaire, documenting your medical and sleep history. There is a link to this questionnaire at the bottom of the page and on the homepage of our website. Your questionnaire will be reviewed by your physician, nurse practitioner or physician assistant before your visit. During your visit, you will discuss your history and current sleep complaints. Depending on the nature of your sleep complaints, you may be asked to keep a sleep diary or to return for testing such as an overnight polysomnogram or multiple sleep latency test. If any testing is done, you will need to return for an office visit to discuss the results and treatment options.

You should bring your insurance card(s) and drivers license and copay with you to your visit. If your insurance requires a referral for you to see a specialist, you will need to bring one, issued by your primary care doctor, at the time of your visit. The front desk staff answers phone calls between 9:00am and 12 noon, and between 2:00 pm and 4:00 pm. Callers for the front desk outside those hours can leave voicemail messages, which are picked up throughout the day.
